Functional Assessments
Evolve Well Health offers functional, developmental and behavioural assessments to support understanding of how a child or adult’s disability, emotional wellbeing, or neurodevelopmental differences impact everyday functioning.
Assessments focus on strengths, support needs, and lived experience across daily life, relationships, communication, emotional regulation, and participation in home, school, and community settings.
Referrals are welcomed from GPs, paediatricians, allied health professionals, schools, and community services. Assessments are offered by appointment and may be completed as a standalone service or as part of ongoing nursing or behaviour support.
Assessments may be used to inform:

Care planning and therapeutic support

Positive Behaviour Support (PBS)

Collaborative planning with families, GPs, schools, and allied health professionals

Supporting evidence for funding or service access where appropriate
Assessments may include:

Functional Behaviour Assessment (FBA)

Adaptive functioning assessment using standardised tools (e.g. Vineland‑3)

Developmental screening to support early identification (e.g. Brigance, ASQ-SE)

Comprehensive biopsychosocial nursing assessment
Assessments are provided within a Registered Nurse & Credentialed Mental Health nursing scope and in alignment with relevant practice and service standards.
Functional and developmental assessments do not determine eligibility for the NDIS or other funding schemes. Where appropriate, reports may be used as supporting evidence as part of a broader application, review, or care planning process.
Fees are discussed prior to commencement and are based on time required, complexity, and purpose of the assessment.
Functional and behavioural assessments are typically completed over several hours and may be provided as a standalone service or as part of ongoing support.
